COC's Universal Closed-Loop Oncology Nurse Navigation Program helps patients move through consultation, diagnosis, treatment and follow-up without becoming lost between services.

Universal Closed-Loop Navigation
Every oncology patient stays connected to the care pathway
Nurse navigation links patients with the appropriate clinical teams, helps coordinate next steps and provides a consistent point of contact across the oncology journey.
1. Entry Navigation The navigator receives the oncology patient, reviews immediate needs and helps organize the first steps in care.
2. Specialist Consultation The patient proceeds to the appropriate oncology specialist and diagnostic services.
3. Return to Navigator Before leaving, the patient returns to navigation so recommendations and next steps can be closed and coordinated.
4. Treatment Coordination Navigation supports appointments and movement between Medical Oncology, Radiation Oncology, Surgery, Imaging, Pathology and supportive services.
5. Longitudinal Follow-up The navigator remains connected with the patient as treatment progresses and follow-up needs change.

What the navigation team does
The nurse navigator helps create continuity across a complex multidisciplinary cancer pathway and reduces the risk that an important recommendation or next appointment is missed.
